Dog and Cat Heartworm: MyPetED Vet on Heartworm Treatment for Dogs, Cats

By Ernest Ward, DVM

What is heartworm disease in dogs and cats?MyPetED image: This heartworm picture shows adult heartworms in a dog’s heart and nearby vessels.

  • Heartworm (dirofilariasis) is a serious and potentially fatal disease.
  • It’s caused by the blood-borne parasite Dirofilaria immitis, carried by mosquitos.
  • Adult heartworms live in the heart, adjacent large blood vessels, and occasionally in other regions of the circulatory system. Heartworms can live up to 5 years.
  • Female heartworms are 6-14" long (15 - 36cm) and 1/8" wide (5mm), and produce millions of offspring called microfilaria. Male heartworms are about half that size.
  • The disease is much more common in dogs. 1 dog may have as many as 300 worms present when diagnosed. The disease mainly affects dogs that are 2-8 years old.
  • The cat infection rate is about 5-20% of the dog infection rate in the same location, but oddly, 1/3rd of infected cats live indoors only. Cats have far fewer adult heartworms when diagnosed; only 6 or so.

Why does the heartworm life cycle matter?MyPetED image: The heartworm life cycle includes a mosquito, which serves as the heartworm host before infecting a dog or cat.

  • Before the heartworm can invade a dog or cat, it needs an intermediary host (the mosquito). 30 or so mosquito species transmit heartworms.
  • When a female mosquito bites a heartworm-infected pet, the life cycle begins.
  • The female ingests microfilariae during a blood meal, and then the microfilariae develop further for 10-30 days in the gut before entering  mouthparts. At this stage, they are infective larvae and can complete their maturation when they enter a dog.
  • When the mosquito bites a new pet, the infective larvae enter the pet's body. They quickly flow to the heart and adjacent blood vessels, maturing to adults, mating and reproducing microfilariae within 6 - 7 months.

Where do dogs and cats get heartworm disease?

  • Canine and feline heartworm disease is found worldwide, including most regions of the U.S. and some regions of Canada; mainly where mosquitos are common.
  • The highest numbers of reported cases are within 150 miles of the Gulf of Mexico and the Atlantic Ocean coastlines, and along the Mississippi River and its tributaries.
  • The greatest number of cases in Canada occurs around the southern Great Lakes.

How is heartworm disease spread?

  • The disease is carried by mosquitos to and from dogs and cats; not directly from pet to pet.
  • It occurs during mosquito season, which can last all year in warmer climates.

What are the most common heartworm signs in dogs and cats?MyPetED image: Adult heartworms (pictured) can live for 5 years in a dog or cat before causing any symptoms of heartworm disease.

  • The life cycle can take several years before an infected dog or cat shows any symptoms.
  • Sadly, by the time symptoms are seen, the disease has progressed to an advanced stage. This means…
  • Adult heartworms clog the heart and major blood vessels leading from the heart, reducing blood supply to the lungs, liver and kidneys. As these organs begin to malfunction, symptoms include:
    • A soft, dry cough
    • Shortness of breath
    • Weakness
    • Nervousness
    • Listlessness and loss of stamina
    • Becoming faint or disoriented after exercise
    • Congestive heart...
